The library can only hold what gets reported. If you've watched an AI system fail in the real world, a hallucinated policy, a wrongful match, a model making a call it had no business making, send it over. Every submission is read, checked against its sources, and, if it holds up, written into the library, with credit if you want it.

What to include

The more of these you can give, the faster it can be verified. None is required, send what you have.

  • What happened — the AI failure, in a sentence or two.
  • The AI system or product involved.
  • The company or organisation behind it.
  • Roughly when it happened.
  • Source links — a news report, court filing, regulator notice, or screenshots. The most useful part: the library only publishes what can be sourced.
  • Your name, if you'd like credit. Anonymous is welcome too.
